Transition planning and supports

Hi community, I'm trying to learn more about how schools and organizations are supporting students with disabilities with transitions between high school and post-secondary options (college, work-ability, adult programs), as well as supporting adults transitioning from school programs as they turn 22. Any information, big or small, would be greatly appreciated!
